Trip Contract
CHAUFFEUR RESPONSIBILITIES:
1- Chauffeur is to walk the vehicle/bus after each drop.
2- Remove all empty containers and trash.
3- Report any accident/Incident immediately.
FOOD POLICY: NO FOOD – NO DRINKS – NO SMOKING
Confirmation Contract
*VEHICLE AGREEMENT:
This is a Contractual Agreement for the hire of a Chauffeured Vehicle Service. Damages to the vehicle, accessories, or excessive cleaning will result in additional charges, which include the actual cost to replace or repair, plus any down time while being repaired. Contracted client is responsible for all guests. Chauffeur has the right to terminate a trip if a passenger is unruly.
*FEES and REGULATIONS:
FOOD – DRINK – SMOKING ARE PROHIBITED ON ALL VEHICLES – NO EXCEPTIONS. There will be a cleaning surcharge for any excessive cleaning and vomit incidents. Alpha Transportation Services is NOT responsible for lost, damaged, or stolen property. Clients are responsible to check the vehicle upon exiting, however if items are found, Alpha Transportation would do the best to return them at the owner’s expense. Any damage to the vehicles by the clients, will be their responsibility at 100% of the cost of damage plus all down time at the vehicle’s hourly rate.
*DEPOSIT: All deposits are non-refundable.
*ASAP REQUEST: A $25.00 Fee will be added to all last-minute request.
* FUEL SURCHARGE: A 10% fuel surcharge will be added to all reservations including affiliate.
*AIRPORT PICK UP:
International pick-ups have (1:00hr) one hour after the flight has arrived. Domestic flights have (40)forty minutes after the flight has arrived. Additional fees will be at $20.00 for every (15) fifteen minutes thereafter. If waiting time is longer than (1:00 hr) One hour, The reservation will automatically change to hourly.
*SEAPORT PICK UP :
Client has (30) thirty minutes after scheduled pick up time. Additional charges will apply at $20.00 for every (15) fifteen minutes. If waiting time is longer than (1:00 hr) One hour, The reservation will automatically change to hourly.
*TRANSFERS:
WAITING TIME
Waiting time shall begin 10 minutes beyond the scheduled pick-up time:
•10-30 mins is $40
•30-45mins is $60
•45-60 is $80
•over 60mins is $100 per hour
*CANCELLATION POLICY: Vehicles must be cancelled prior the minimum notice of the scheduled pick-up time or will be subject to a service fee equal to the cost of the entire trip if cancelled less than:
Minimum notice for cancellation prior to scheduled pick-up:
SUV – (12:00hrs) Twelve hours – 50% of Estimated Charge
SUV – (24:00hrs) Twenty-Four hours – No Charge
Van – (24:00hrs) Twenty-Four hours – 50% of Estimated Charge
Van – (48:00hrs) Forty-Eight hours – No Charge
Mini-Bus/Mini-Coach (72:00hrs) Seventy-two hours – 50% of Estimated Charge
Mini-Bus/Mini-Coach – (84:00hrs) Eighty-Four hours – No Charge
Motorcoaches, Specialty Vehicles –– (96:00hrs) Ninety-six hours – 50% of Estimated Charge
Motorcoaches, Specialty Vehicles – (144:00hrs) 6 Days – No Charge
All reservation information contained is considered true and accurate unless corrected by notifying Alpha Transportation Services.Clients should contact our office at (561) 372-8777 if they are unable to locate the chauffeur. Failure to do so will result in a 100% charge of the cost of the trip.
*PET POLICY:
Pets are allowed to ride in the vehicles with no additional fees if crated. If they are not crated, please note there will be a fee of $20 per pet (small ) and $40 ( large ). (Client is responsible to walk pet before entering the vehicle, A fee of 250.00 will be charge for pee, poop or vomit. )
*CARE OF VEHICLE
All vehicles are considered «Non-Smoking» vehicles. Client agrees to take care of the vehicle, its components and furnishings and maintain the vehicle in a neat and sanitary condition. At the chauffeur’s discretion, the client agrees to be fully liable for all of the following charges that may occur.
1. $20.00 — each missing or broken piece of glassware.
2. $25.00 — minimum extensive clean-up charge (spills, mud, candy, etc).
3. $50.00 — minimum for detailing and waxing due to sickness outside of the vehicle.
4. $250.00 — minimum for shampooing and disinfecting inside of vehicle due to incontinence and or any other bodily fluids.
5. $100.00 — minimum for each burn hole or tear to the upholstery, carpet or any other vehicle components.
6. $100.00 — minimum for each and every act of vandalism.
7. $50.00 — for each violation of the smoking rule as described above.
* TOLLS AND ADDITIONAL STOPS WILL BE ADDED AT THE END OF ALL TRIPS. (If reservation requires the above) ***

QUOTATION POLICY: All quotes are only honored for 5 DAYS max. Quotes are not guaranteed as booked unless you have confirmed to BOOK any reservation(s) and a $200.00 NON -REFUNDABLE deposit is required if amount is over $500.00
Affiliate contract
PLEASE CONFIRM THIS RESERVATION UPON RECEIPT.
ALL CHANGES OR SPECIAL REQUESTS BY THE PASSENGER, MUST BE PRE-APPROVED BY ALPHA.
By accepting this reservation, Affiliate agrees to represent Alpha Transportation in a manner consistent with the highest industry standards, and not to distribute any printed material.
1-No signage or Business cards other than those representing or issued by ALPHA Transportation Services.
2- Vehicles must be clean / Black on Black (unless authorized ). contain bottle of water, free of smoke or strong odor.
3- Fifteen (15) minutes spot time on each job required.
4- Driver name and cell # must be provided to ALPHA Transportation Services prior to the start of each job.

For proper payment, please bill or receipt ALPHA by fax or email within 48 hours of the completed reservation.
